What Your Cholesterol Numbers Actually Mean
A standard lipid panel gives you four numbers. Most patients focus on one of them, misread a second, and ignore the other two entirely. Here is the full picture.
Every year, thousands of patients receive their lab results, see the word "borderline" next to a cholesterol figure, and do one of two things: either they panic and call the office, or they file the paper away and try not to think about it. Both responses suggest the same underlying problem. The number means very little in isolation. What matters is the complete picture of what your lipid panel is showing, how those numbers interact with each other and with your other risk factors, and what, if anything, actually needs to change.
September is National Cholesterol Education Month, which seems like an appropriate occasion to work through what a standard lipid panel is actually measuring and what the results tell a physician about your cardiovascular risk.
The Number Everyone Knows LDL: Low-Density Lipoprotein
LDL is the number most people focus on, and for good reason. Low-density lipoprotein is the primary mechanism by which cholesterol deposits build up in arterial walls. When LDL is elevated over a sustained period, it contributes to atherosclerosis: the slow accumulation of plaque that narrows arteries and raises the risk of heart attack and stroke. The National Lipid Association notes that the relationship between LDL and cardiovascular risk is cumulative, with benefit observed at progressively lower levels.
The target LDL is not the same for every patient. For someone with no history of cardiovascular disease and no significant risk factors, a level below 130 mg/dL is generally acceptable. For someone with diabetes, hypertension, or a prior cardiac event, the target may be below 70 mg/dL. This is why reading your LDL in isolation, without understanding your individual risk profile, is not particularly useful. The number needs context.
The Number People Misread HDL: High-Density Lipoprotein
HDL is the one case in cholesterol medicine where higher is better. High-density lipoprotein functions as a kind of reverse transport system: it carries cholesterol away from arterial walls and back to the liver for processing. Higher HDL levels are associated with lower cardiovascular risk, and low HDL is itself a risk factor even when LDL looks normal.
For men, an HDL below 40 mg/dL is considered low. For women, the threshold is 50 mg/dL. An HDL above 60 mg/dL is generally considered protective. Research published in a 2024 PMC review confirms that both low HDL and very high HDL are independently associated with increased cardiovascular risk. The mistake patients most commonly make is celebrating a normal total cholesterol number without noticing that their HDL is too low to be doing its job effectively.

The Number Most People Ignore Triglycerides
Triglycerides are a type of fat in the bloodstream, and elevated levels are independently associated with cardiovascular and metabolic risk. They are also one of the most diet-responsive figures on the panel: refined carbohydrates, sugar, and alcohol can drive triglycerides up significantly, sometimes more dramatically than dietary fat does.
According to the American Heart Association, normal triglycerides are below 150 mg/dL. Levels between 150 and 199 are borderline high. Above 200 begins to warrant clinical attention, and levels above 500 carry risk of pancreatitis in addition to cardiovascular concerns.
The Least Useful Number Total Cholesterol
Total cholesterol is the sum of LDL, HDL, and a fraction of triglycerides. It is the figure most commonly reported in general health summaries, and it is the least informative of the four. A person with high total cholesterol driven primarily by high HDL may actually have a favorable risk profile. A person with normal total cholesterol and low HDL may be at significantly higher risk than their headline number suggests.
The ratio of total cholesterol to HDL, sometimes called the cardiac risk ratio, gives a more useful signal than total cholesterol alone. A ratio below 4 is generally favorable. Above 5 warrants attention. Your physician can calculate this from your panel and put it in the context of your other risk factors.
What to Do With the Numbers When Results Warrant Action
Lifestyle modification is the first-line response to most lipid abnormalities. Reducing saturated fat and increasing fiber, particularly soluble fiber from oats, legumes, and vegetables, has a clinically meaningful effect on LDL in most patients, consistent with the 2018 AHA/ACC cholesterol management guidelines. Regular aerobic exercise raises HDL and lowers triglycerides. Weight loss, if relevant, improves the entire panel. Reducing alcohol intake lowers triglycerides directly.
When lifestyle changes are insufficient, or when a patient's cardiovascular risk is high enough that waiting is not clinically appropriate, statins remain the best-evidenced medication class for LDL reduction. The decision about when to treat, what to treat with, and how aggressively to target a given number is a clinical conversation that depends on your full risk profile, your other health conditions, your family history, and your own values about treatment.
