We verify your insurance

Our team confirms your benefits and coverage directly with your insurance plan. You'll hear back from us by the next business day. You always have the option to use our self-pay rates.

03 / FILL NEW PATIENT FORM

Complete your New Patient forms

A short process covering the basics: Your demographics, contact information, reason for visit, and a photo of the front and back of your insurance card.

A few things to know first.

We want to make sure we are the right fit before you spend time on paperwork.

Our providers see patients 18 and older.
We do not accept HMO plans at this time.
We do not accept workers' compensation claims.
We do not accept auto-accident claims.
02 INSURANCES

Insurance we accept.

Medicare (Original), most Medicare Advantage PPO plans, Tricare, and most major commercial PPO plans: BCBS, UnitedHealthcare, Aetna, Cigna, Humana, and others.

Patients are encouraged to verify network participation directly with their insurance company prior to scheduling an appointment.
03 Self-pay

No insurance? You can still come see us.

If you're paying out of pocket, see our transparent self-pay pricing. No surprise bills.

Questions we hear a lot.

No. Our providers see adults 18 and older.

No. We accept PPO plans only, including Medicare Advantage PPO plans. If you are on an HMO and are not sure, call 850-848-9500 and we will confirm.

Our scheduling team will call you after your paperwork is received and reviewed. Most new patients are seen within the week of that call.

Yes. We coordinate with them, request records, and share visit notes.

Yes. Messaging, records, and refill requests live in our Healow portal. Access is sent after your first visit.
