Medical Records Request Information



As a patient of Trinova Medical, you can easily request your medical records through our partner, MediCopy | MRO. We've partnered with MediCopy to ensure a secure, HIPAA-compliant, and efficient process for all release of information requests.


Here's how to request your medical records if you are a patient:


  • Online Request: The quickest way to request your records is by visiting MediCopy's patient page at MediCopy.net/Patients

  • Authorization Form: You can also obtain an authorization form directly from our office.


  • Email Address: Please be sure to provide your email address on the authorization form to help expedite the process and delivery method.


Third-Party Requesters:

Important Information:


  • Cost: Records sent via email or fax, or to another provider (doctor-to-doctor requests), are complimentary.  If records are delivered by mail, delivery fees may apply. For requests over 80 pages, a fee of $0.03 per page plus postage may be charged.


  • Processing Time: Requests are typically processed within two (2) business days after receipt by MediCopy.
