What Trinova 65+ is.

Trinova 65+ is our primary care program for adults 65 and older. Same practice, same team, shaped around Medicare. It covers what Medicare expects of a good primary care provider — your Annual Wellness Visit, preventive screenings, chronic condition check-ins, medication reviews — plus a few things it does not. A provider on call after hours, any day. Specialists and hospitals coordinated for you, so you are not the one relaying messages between them. Billed through Medicare like any primary care visit. No membership, no subscription, no concierge fee.

03 CCM

Care between the visits, not just at them.

If you qualify for Medicare's Chronic Care Management program, you are enrolled on day one. Chronic Care Management is a Medicare program for anyone living with two or more long-term conditions. Your care team reviews your chart between visits, coordinates with specialists, and reaches out when labs, medications, or symptoms need attention. Most practices make it opt-in and pass the monthly copay to the patient. We make it the default and write the copay off.

04 ELEGIBLITY

Who can join.

Adults 65 and older with traditional Medicare and Medicare Advantage Plan (PPO). Managing a chronic condition or simply looking for proactive primary care, both fit the program. No membership fee. No subscription. No waiting list.

Questions we hear a lot.

No. Trinova 65+ uses standard Medicare billing. There is no subscription, retainer, or concierge fee.

No. Chronic Care Management is a Medicare program with a monthly copay in most practices. We write that copay off for every qualifying patient. You sign the consent once at onboarding.


Yes. We coordinate with them, request records, and share visit notes so nothing falls through the cracks.

Usually within the week. After that, established patients can typically be seen same or next day for acute issues.

Yes. Follow-ups, medication refills, and results reviews work well by phone or video. Anything needing a physical exam stays in person.
