The Role

Position Overview

We are seeking an experienced, highly organized Value-Based Care Coordinator to help us achieve our end-of-year quality and risk-adjustment goals. This is a self-directed, hybrid role responsible for chart review and HCC coding, quality-registry management, and direct patient outreach to close care gaps. It is an excellent fit for an experienced care professional — such as a Medical Assistant, LPN, certified coder, HEDIS/quality abstractor, or care coordinator — with value-based care, population health, or chronic care management experience who can manage a project independently while collaborating closely with our clinical team. This is a seasonal position — now through the end of the year — with the potential to extend into 2027 based on performance and practice needs.


Day-to-Day

What You'll Do

• Prepare and maintain payer worklists and quality registries in eClinicalWorks (eCW) across key measures: HCC/risk adjustment, blood pressure control, diabetes (A1C), diabetic retinal exams, statin therapy, and Annual Wellness Visits
• Review patient charts and accurately capture and reconcile HCC diagnosis codes as supported by clinical documentation and practice protocols
• Conduct high-volume patient outreach by phone and text to close care gaps — scheduling visits, requesting outside records (such as retinal exam results), and coordinating follow-up
• Support blood pressure control efforts: identify patients who are uncontrolled or overdue, facilitate home blood-pressure monitoring and remote patient monitoring (RPM) enrollment, and route clinical documentation to the RN and APRN team
• Support diabetes (A1C) control outreach and statin-therapy gap closure in partnership with providers
• Assist with Annual Wellness Visit and Health Risk Assessment (HRA) completion, escalating any clinical concerns to the provider team
• Maintain accurate gap-closure documentation and registries, and support CCM/APCM billing workflows
• Collaborate closely with RNs, APRNs, and front-desk/scheduling staff


Requirements

Qualifications

• Clinical background sufficient to review charts and engage patients confidently — e.g., Medical Assistant (CMA/RMA), LPN, certified coder (CRC/CPC), HEDIS/quality abstractor, or experienced care coordinator
• Experience in value-based care, population health, HEDIS quality measures, HCC/risk-adjustment coding, or chronic care management (CCM) — required
• Proficiency with eClinicalWorks (eCW) strongly preferred
• Demonstrated ability to work independently and manage a project with minimal supervision
• Excellent communication skills and comfort with high-volume patient outreach by phone and text
• Strong organizational skills, attention to detail, and comfort working with registries and spreadsheets
• Familiarity with HCC coding and risk adjustment; Certified Risk Adjustment Coder (CRC) credential a plus
• Professionalism, reliability, and a collaborative approach


Compensation

Schedule, Location & Compensation

• Competitive pay: $26–$30 per hour, commensurate with experience
• Part-time, approximately 20–28 hours per week
• Hybrid — a combination of in-office (Pensacola, FL) and remote work
• Seasonal through December 2026, with the potential to extend into 2027
