Why patients switch

Common reasons we hear.

Most of the patients who switch to us tell us the same things. Their previous practice took three weeks to schedule. They saw a different provider every visit. Refills got missed. Specialist referrals went into a void. The doctor was bought by a hospital system and access changed overnight.

  • Wait times stretched past two weeks
  • Different provider every visit
  • Doctor retired or relocated
  • Practice was acquired by a hospital system
  • No follow-through on labs or referrals
  • Phone calls not returned

Three steps to switch

We do most of the work.

Call or text us

Call (850) 848-9500 with your Medicare or MA card. We verify benefits and confirm we are accepting new patients on your plan.

Sign records release

We send a release form to sign and we request your records from your previous PCP. You do not have to chase them down.

First visit

Most new patients are seen within the same week. We walk in already knowing your medication list and history.

The process

Original Medicare vs Medicare Advantage PPO.

If you have Original Medicare

You can choose any provider who accepts Medicare. No formal PCP designation required. Walk in as a new patient, and we take it from there.

If you have Medicare Advantage PPO

Update your PCP to Trinova in your plan's member portal, or call your plan's member services line. We give you the exact provider ID.

Common questions

Frequently asked questions.

Do I have to tell Medicare I am changing doctors?

Not for Original Medicare. For Medicare Advantage PPO, you may need to update your PCP designation in the plan's member portal.

Will my Medicare benefits change if I switch?

No. Your benefits travel with your card, not with your provider.

How long does the records transfer take?

Typically 7 to 14 business days, sometimes faster if your previous provider uses an electronic record-sharing network.

Do I need a referral to start care at Trinova?

No referral is needed.

Can I switch mid-year on Medicare Advantage?

Yes, you can change your PCP within your existing Medicare Advantage plan at any time.

Will I have to start over with my prescriptions?

No. We continue your existing prescriptions and review them at your first visit.

What if I want to keep some specialists I already see?

You can. We coordinate with them and pull their records into your file.

Will my new doctor know my history?

Yes. We request and review your records before your first visit.

How quickly will my first visit be scheduled?

Most new patients are seen within the same week.

What do I need to bring to my first visit?

Your Medicare card, any Medicare Advantage card, photo ID, and a list of your current medications.
